A Case of Waldenstrom's Macroglobulinemia, without bone marrow involvement

Abstract

|
Waldenstrom's macroglobulinemia is a n uncommon dysproteinemia with diverse clinical manifestations characterized by fatigue, bleeding diasthesis, hepatosplenomegaly, lymphadenopathy, lymphadenopathy, ocular changes, and neurologic symptoms.
Hyperviscosity syndroms may develop as a complication of macroglobulinemia. We had experienced a case of Waldenstrom's Macroglobulinemia, who visited our outpatient department of the internal medicine due to dizziness and frequent nasal bleeding.
Physical examination revealed enlarged Lt. Cervical lymph node and mild hepatosplenomegaly Serum protein electrophoresis showed the sharp M-peak in the gamma-globulin region (46.1%). Ig., monoclonal gammopathy. And by immunoelectrophoresis of the
serum
& urine protein, IgM kappa type macroglobulinemia was confirmed.
Bone marrow aspiration and biopsy showed nonspecific findings except mild lymphocytes infiltrations. But cervical lymph node biopsy revealed plasmacytoid lymphocyte and histiocyte infiltrations.
She was treated with supportive therapy only and we are following up clinical status till now.
